In-Home Follow Up Consult

Follow up consultations include evaluation of current symptoms, physical assessments and management of the current care plan to address changing needs/outcomes. It’s important to keep in good contact with your LC to make sure that you are happy with your plan of care. Allow 60-90 min for follow up appointments.
